Bon Secours Mercy Health Licensed Practical Nurse (LPN) Central Intake - Home Care in Youngstown, Ohio

Primary Function/General Purpose of Position

The Central Intake Licensed Practical Nurse, LPN registers and schedules all Home Care patients/clients. This role coordinates the intake process with physician offices, emergency departments, and other healthcare facilities and ancillary areas. They assist with insurance verification and related support duties. In addition to central intake processing, the Central Intake LPN performs necessary clinically related functions to support the intake/admission process.

Essential Job Functions

  • Welcomes patients/clients by offering service, assistance, and determining the reason for their service to facilitate activities to meet their needs

  • Reviews clinical information, troubleshoots complex patient referrals, and discusses with appropriate personnel to ensure referral is appropriate. Enters and obtains orders for referrals

  • Completes the insurance verification process

  • Assigns and coordinates teams for all admissions, accommodating patient and physician requests. Works collaboratively with healthcare facilities and physician offices in support of the admission process

  • Accurately informs team of expected discharge date of patients, works collaboratively with all areas in support of the scheduling process, and acts as a patient resource in the scheduling of follow-up procedures/appointments

  • Performs system maintenance and clerical duties, including merging patient information when two accounts exist, loading new doctor and facility information into system. Running and updating reports as needed and validating physician licenses

  • Performs home visits and other duties as assigned

Licensing/Certification

Current Licensed Practical Nurse (LPN) from the state of employment of LPN applicant (required)

BLS Basic Life Support - American Heart Association (required)

ACLS Advanced Cardiac Life Support (may be required for specialty areas)

PALS Pediatric Advanced Life Support (may be required for specialty areas)

NRP Neonatal Resuscitation Program (may be required for specialty areas)

Current State Issued Driver’s License (preferred)

Education

None

Work Experience

1 year of clinical patient care experience (e.g., PCT, CNA, LPN) in an acute care setting, long-term care facility, or physician practice (preferred)

Training

EPIC Electronic Health Record (EHR) training (preferred)

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.

Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.​

Working Conditions
