**[Job Overview: [The Licensed Practical Nurse (LPN) is responsible for providing direct nursing care to In-Home Services clients according to the individuals plan of care set forth by DHSS or listed in the care plan for self-paid clients. The LPN provides services in the clients home, completes service and client documentation, and maintains open communication with the supervisor to ensure the health, safety and well-being of the client.

Is responsible for timely documentation of services in both the consumer database and EVV system.

Maintains open communication with the IHS Coordinator and/or the Director of Home and Community Based Services and Compliance, regarding concerns or issues related to the client's health, safety and well-being.

Be available for case conferences and occasional staff meetings.

Administers In-Home Services in accordance with all regulations and guidelines of Department of Health and Senior Services and empower: abilities.

Office/administrative duties.

Perform authorized nurse visits, including the following tasks:

Monitor skin condition

Set up and administer prescribed oral medications

Nail care for the diabetic client or client with other contraindicating condition
