University of Rochester Social Worker in Rochester, New York

GENERAL PURPOSE:

The OB/GYN social worker is responsible for providing clinical services to patients who receive care at UR Medicine Gender Wellness, Obstetrics & Gynecology or admitted to inpatient OB/GYN units. Clinical services are also provided to the families of and designated support people of these patients.

RESPONSIBILITIES:

  • Conducts risk screening - completing psychosocial assessments for those with identified risks. Psychosocial risk factors may include; unplanned pregnancy, adolescent pregnancy, adoption, substance use, behavioral health issues, domestic violence, child maltreatment/neglect concerns, human trafficking, medical complications in pregnancy, pregnancy loss, and inadequate resources.

  • Based on assessment, provides interventions to patients and families in order to address identified psychosocial needs. This includes linkage to medical and community-based services & resources, supportive counseling around complicated medical & social issues. The social worker also provides discharge planning for patients, assuming the primary coordinator role when there is child protective involvement, interpersonal violence, alternate discharge of a minor or there is a transfer to another facility. Documents social work activity in patient’s medical record and identifies time spent.

  • Rounds with and collaborates with members of the health care team around matters of patient care and discharge planning. Identifies risk management issues and communicates them to the OB/GYN Senior Social Worker. Collaborates with other social workers to ensure continuity of care across clinical settings. Provides backup social work services in the absence of the assigned social worker.

  • Serves as liaison to health and social service agencies involved in the care and support of patients. Participates in activities of the Social Work Division, OB/GYN departments and Hospital including mandatory training, UHS reviews, in – service trainings and time reporting. Meets all expectations of employment including maintaining NYS licensure.

  • May serve on Social Work and OB/GYN committees. May participate in teaching of Social Work Division and Hospital staff regarding psychosocial issues impacting women. May supervise a social work student placement.

Other duties as assigned

QUALIFICATIONS:

  • Must hold a master of social work degree from an accredited institution.

  • Experience in the provision of services to women and teens of reproductive age and or within a health care setting is preferred.

  • Foundational skills of the master prepared social worker is required. Ability to work with diverse populations and to offer services regardless of personal beliefs concerning reproductive options preferred.

  • NYS licensure required within 18 months of hire and maintained thereafter required.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
